Geriatric Medicine Associates

Geriatric Medicine Associates is a physician owned group that has been dedicated to the care of elderly and frail elderly for over 29 years. We were founded by Dr. Alan Lazaroff (Current AGS board member) who had a vision for providing high quality, comprehensive primary care for seniors. We have a robust team that work together towards the goal of coordinating the healthcare of this population. We have participated in the Pioneer ACO, Medicare Shared Savings Program, and the Comprehensive Primary Care Plus program all connected with the Center for Medicare Services.

Geriatric Medicine Associates is a founding member of Physician Health Partners http://www.phpmcs.com/, which manages our clinics and also provides support for many primary care physicians in the greater Denver area. Our partnership with Physician Health Partners improves our quality, care delivery, and workflows by being an active participant in our Quality Improvement teams.  Physician Health Partners also keep us informed on reporting and payment issues that are important component of a successful geriatric practice.

Geriatric Medicine Associates understands the challenges in providing high quality, cost effective care to our population. We understand the pressures on primary care physicians to provide “value”. Our structure, partnership with St. Anthony’s hospitals and Physician Health Partners, and the culture of our group allow us to meet those challenges and create sustainable fulfilling careers.
